Simply return all forms and deposits (check or money order only - payable to US Federal Credit Union) listed in the checklist below to one of our convenient locations or mail to:

Application Checklist:

             - Completed Business Services Application
             - Completed Resolution of Authority (if applicable)

- $5 minimum deposit to open a Share Savings Account (required)

- $100 minimum deposit to open a Checking Account (if applicable)

- Copies of current government-issued ID for each authorized user
- Copies of all necessary government-filed paperwork:
        - Sole proprietorship* - Certificate of assumed name with proof of publication
        - Partnership* - Partnership Agreement, Articles of Organization (LLP only),
                 Certificate of Organization (LLP only), Statement of Qualification (LLP only),
                 Certificate of assumed name with proof of publication (if applicable)
        - Limited liability company* - Articles of Organization, Certificate of Organization
        - Corporation* - Articles of Incorporation, Certificate of Incorporation
        - Non-profit organization** - 501(c)(3)

With completed paperwork and all necessary documentation provided, your application can usually be processed the same day it is received at our Business Services office located at the above address. Once opened, you are free to conduct business at any US Federal Credit Union location.
