• Account Closure Form - Close your accounts elsewhere by mailing this completed form to previous institution(s).
• Direct Deposit/Automatic Withdrawal Identification Checklist - Identify any companies you have authorized to automatically withdrawal funds from your accounts.
• Direct Deposit Change Form - Update your direct deposit to be sent to your US Federal account.
• Automatic Withdrawal Change Form - Update automatic withdrawals to your US Federal account by sending each company making withdrawals this completed form.
